Care Team Coordinator (LPN) - $1,500.00 Signing Bonus
Location:Bellingham, WA

This position is a full-time, non-exempt position, supporting the Family Medicine.  Current work schedule: Monday – Friday, within the hours of: 7:30-5pm; with rotation of 1 Saturdays, 8:00-4pm.  The schedule may vary, as needed.   

Care Team Coordinator Opportunity Highlights:

  • The Care Team Coordinator assists in daily patient management and is part of a multidisciplinary patient care team providing and coordinating medical care in an outpatient setting following a patient-centered, primary care model. 
  • UCNW manages patient care using a team-based approach in our interactions with patients and working to achieve stated objectives and outcomes.
  • This position will work closely with a multidisciplinary team consisting of 2 medical providers, 1 RN, & 2-3 MA’s.  We provide an innovative and integrated approach to care which includes working with Behavioral Health Counselors and Behavioral Health Case Managers.

 Ideal Candidate Offers:

  • Completion of an LPN program from an accredited institution.
  • Washington State LPN License and current BLS.
  • At least two (2) years’ experience within primary care outpatient clinic.
  • Must demonstrate communication skills, problem-solving skills, a strong knowledge in clinical support practices and procedures and have a “team” concept attitude. 
  • Strong knowledge of medical equipment, nursing procedures, documentation and patient education. 
  • Must be accurate and expedient in entry data into patient records and proficient in using computers EMR & MS Office products (i.e. MS Excel, Outlook, and Word), faxes, telephones, and copy machines.  Must be able to organize and prioritize work load. 
  • Must maintain an effective and positive professional working relationship with staff and patients, always, and have a team concept attitude. 
  • Ability to understand and respond effectively and with sensitivity to special populations served by UCNW.  Special populations include, but are not limited to, those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, & all others.
