Case Manager in Specialty Behavioral Health/Community Mental Health
Location:Bellingham, WA

We believe mental & emotional health is an important part of overall health and wellness.  Our behavioral health staff work closely with patients and their Primary Care Providers to help them meet their health care goals.  As a Case Manager, you would facilitate care coordination and referrals to wrap-around services that support clinical care and enrollment into benefit programs such as transportation, housing, food, senior services, long term care facilities, dental resources, substance abuse services and crisis support programs. You would reach out to patients to help improve outcomes, increase compliance, improve recall and retention, enhance advocacy, and develop cooperative relationships with other health, chemical dependency and social services agencies.  You would be part of a collaborative care team that includes therapists, consulting psychiatrists, nursing & medical assistant staff, primary care providers and/or other clinic staff in a behavioral health setting.  This role is key in providing patient-centered, team-based care with a focus on patients with behavioral health, substance abuse, and chronic conditions.

The ideal candidate will possess the following, but candidates with other experiences are welcome to apply:

  • Bachelor’s or Associate’s degree in psychology, social work, counseling or a related field

  • Agency Affiliated Counselor Registration through the WA State Department of Health or ability to obtain one (see

  • Experience working in a community health center or similar organization with patients who have co-occurring mental health, substance abuse, and physical health problems

  • Familiarity with social services in Whatcom County

  • Demonstrated success in working with and relating to underserved &/or transient individuals from diverse cultures, backgrounds & socio-economic levels, treating them with sensitivity & respect
