Summary:

Upper Great Lakes Family Health (UGLFH) is seeking a Quality Coordinator to assist with clinical quality improvement initiatives of all service areas and provide leadership in population health management and continuity of care, efficient use of electronic health record, Patient Centered Medical Home requirements, Uniform Data System reporting, and other clinical quality-related activities.    

Responsibilities, including but not limited to:

  • Monitoring day-to-day PCMH model processes and submission of materials required by accrediting organizations.
  • Conducts research, develops, and evaluates work plans, revising plans accordingly based on impact to clinical services.
  • Performs pre- and post-assessments on clinical projects to ensure efficacy and achievement of project objectives; and conducts ongoing CPI review.
  • Assists the Chair of CPI Committee, Director Quality and the management team in implementing, maintaining, and improving the CPI program plan, including the schedule of CPI activities and quality work plan.

Job Qualifications:

GENERAL PROFESSIONAL DEVELOPMENT

  1. Organization: Displays basic organizational skills to organize own work.

  3. Analysis: Uses inference and logic to draw conclusions and communicates opinions effectively.

  5. Communication: Compiles and analyzes information prepared in written form, including correspondence, reports, articles, or other documentation.
  6. Expertise: Must know all aspects of Quality, sufficient to effectively train or instruct others, and to serve as a resource for employees.

 

PROFESSIONAL AND TECHNICAL KNOWLEDGE

Possesses the equivalent of formal training in nursing or healthcare administration, generally obtained in the form of a bachelor’s degree.

 

  1. Demonstrates an understanding of FQHCs.
  2. Acquires and maintains expert knowledge of National Committee for Quality Assurance (NCQA) and Blue Cross Blue Shield of Michigan (BCBSM) PCMH standards, policies, processes, and application requirements.

 

LICENSES & CERTIFICATIONS

  1. Certified Professional in Healthcare Quality (CPHQ)
  2. Certified Content Expert (PCMH-CCE)

 

TECHNICAL SKILLS

  1. Microsoft Office Suite to create correspondence, tables, charts, graphs, spreadsheets, and presentations.
  2. Azara to identify gaps in care.
  3. Adobe Acrobat to create and manipulate forms.
  4. Electronic clinical systems, including EHR and scheduling systems in use at the health center.

 

COMMUNICATION SKILLS

  1. Effectively communicates information during informal and formal video presentations.
  2. Effectively conveys complex information to varying audiences, verbally and in writing.
