Summary:

Upper Great Lakes Family Health (UGLFH) is seeking a Care Manager to work closely with patients, their family members, and their health care teams to coordinate health care services to optimize patient outcomes.

Responsibilities

  • Coordinate health care services and monitors clinical results to optimize patient health outcomes.
  • Ensures the continuity of care which extends beyond the UGL practices(s).

Job Qualifications:
  • Active unrestricted State of Michigan Registered Nurse (RN) license
  • Evidence of life support certification.
  • Proficient in Microsoft Office Suite, Adobe Acrobat.
  • Experience utilizing an electronic health record system.
