Upper Great Lakes Family Health Center in Hancock, MI seeks Physician for Internal medicine/geriatric physician on all adults 18 and over. Responsible for delivery of quality medical care to patients of the Center and functions as a member of the multi-disciplinary health care team within the scope of his or her medical practice. Provide clinical services and clinical oversight under the supervision of the Medical Director. Maintain clinical expertise and competency (to include age specific competency) as well as demonstrates thorough knowledge of procedures to deliver quality primary health care. Provide clinic sessions, as scheduled, in direct patient care at clinic site, including extended hours clinics and other outreach locations as direct and in accordance with assigned full-time equivalence (FTE). Employ non-surgical methods and procedures to diagnose and treat patients’ medical conditions. Consult with patients to understand their symptoms and health concerns and diagnose and treat acute illnesses including bacterial or viral infections and chronic diseases and conditions including hypertension, heart disease, or diabetes. Prescribe or administer medication, therapy, and other specialized medical care to treat or prevent illness, disease, or injury. Administer immunizations, explain procedures, and discuss test results or prescribed treatments with patients. Internal medicine physicians maintain detailed patient notes and write patient referrals when necessary. Research the testing and development of new medications, treatment methods, or procedures to prevent or control illness, disease, or injury. Geriatric doctors, also called geriatricians, specialize in caring for aging adults who often have complex medical issues. Focus particularly on keeping patient functional and help maintain their quality of life. Examine, diagnose, treat and/or refer patients assigned to physician’s panel, on an in-patient and out-patient basis (as per individual employment agreements); prescribe pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions. Refer clients for appropriate specialty care services, lab and x-ray and any other ancillary services that are appropriate for patient’s management and care. Review all lab and x-ray reports for patients under his/her care on a timely basis and makes medical management decisions appropriately. Follows appropriate standards of care for each patient. Provide continuity of care to center patients on his or her patient panel and other patients as assigned, including routine in-patient, outreach services, home-visits and nursing home / foster home care to center patients, and school-based clinics to include injury assessment and treatment and injury prevention and education, as needed. Perform emergency on-call on a scheduled basis with other physicians within the group and if required by the local hospital(s). Participates in after-hours phone coverage for UGLFHC patients if needed. Maintain a problem-oriented health record for each patient seen in accordance with the Center’s established Health Information Management policies and procedures. Ensure that patient and family/significant others are educated about diagnosis and the plan of care. Act as a resource for facilitating communication when necessary. Participate in specified health promotion, education and/or prevention programs as needed (i.e. diabetes collaborative/health fairs, school-based injury prevention and concussion education, etc.); attend and participates in clinic meetings / departmental meetings and other clinic functions as required by the Center. Assumes responsibility/accountability for delegating, directing, and supervising patient care activities. Geriatric doctors understand caregivers’ roles and work with family members, too. Geriatric doctors diagnose and treat issues that affect older adults. Their patients often have one or more chronic health issues. Some of the job duties will include but are not limited to the following: accountable for coaching, mentoring, and role modeling to achieve the highest quality patient care in a safe, cost effective and customer focused environment. Participate in medical student, and resident precepting and education. Practice with a multi-disciplinary approach alongside other service providers to optimize patient experiences and outcomes. Provide ongoing effective supervision to unit employees and monitor case management activities. Clinical responsibilities will include managing a panel of patients in primary care as well as participating in call coverage with other members of the team. Review labs, ancillary tests, studies, and consultations from specialists and follow up. Figure out their priorities for healthcare. Create care plans. Review and manage all medications. Coordinate care with other health providers. Provide palliative and end-of-life care. Participate in the analysis of community health problems and assists in the development of the community health care plan. Participate in Compliance and Performance Improvement activities for continuous care quality improvement and risk management purposes. Participate in the review and development of patient care policies, including health maintenance schedules and continuity of care. Participate in the review and development of protocols and standing orders. Implement the plans, procedures and protocols for the medical team. Serve as a resource and positive, proactive leader for the department. Accept responsibility for remaining current with medical and health care trends and information that impact medical practice. Lead the health care team in identifying, evaluating, and prioritizing patient and family needs to develop a plan of care. Participate in the evaluation of products, procedures, and patient care standards. The Physician will train, mentor and supervise 4th year medical students completing rural rotations who are Medical Assistants and Certified Medical Assistants and clinical support staff and clinical staff. Physician will not have authority to hire or fire any of the students or clinical staff, they would not be directly reporting to the Physician. They would work alongside the Physician assisting patients, and the Physician would purely be training, mentoring, and supervising.

Travel:  15% travel required throughout the Upper Peninsula between clinic sites and outreach patients.

Job Requirements:  Must possess a Medical degree (M.D. or D.O.). Must have completed all requirements of a 3-year residency program in Internal Medicine and a fellowship program resulting in Board Eligibility with American Board of Internal Medicine. Must possess Basic Life Support (BLS) and be licensed from the Michigan Licensing Board to practice as a physician in the State of Michigan.
