Job Summary:
The Caseworker in the Rockland Community Based Services (CBS) program is responsible for the Care Coordination of Targeted Case Management-HH Cases or the Children’s Waiver-HH cases. This includes assessment of new referrals, outreach to gain referrals and coordinating medical and mental health services with assigned Health Home.

Duties & Responsibilities:

THIS POSITION HAS A $4,000 SIGN-ON BONUS

The Caseworker will conduct mandated monthly client contacts, attend necessary appointments with clients, and develop the client’s Plan of Care by the required due dates to include the Department of Health (DOH) required 10 Care Plan elements. The Care Manager will complete the Child and Adolescent Needs and Strengths (CANS) certification, follow all Health Home policies and procedures, and perform other related tasks.

Education & Experience

Bilingual Spanish/English is required. A bachelor’s plus 2 years of experience providing direct services to Severely Emotionally Disturbed (SED) children and adolescents or providing linkage to services for children with one mental health diagnoses OR a master’s plus 1 year of experience is required. Bilingual Spanish/English is required.  A valid non-restricted NYS driver license is required.
Physical Requirements:

While performing the duties for this job, the employee is regularly required to communicate clearly and effectively.  The employee is required to stand; use mobility, handle, feel; and reach. The employee is occasionally required to sit and stoop, kneel or crouch.  The employee must occasionally lift and/or move up to 25 pounds.
