Job Summary:
GENERAL SUMMARY
Provide nursing service, documentation and record review. Oversee medical information; verify follow up and documentation; chart reviews, appointments; chart maintenance

Duties & Responsibilities:

This position has a $3,000 sign on bonus

Chart review, completion of routine work, documentation, schedule appointments, provide comforting bedside care, administer medication, take all vital signs, collect samples, provide wound care, help coach staff as necessary, etc.

Job Type: Full-time

Salary: $58,000.00 per year

Education & Experience
EDUCATION & EXPERIENCE

NYS LPN license required with at least 2 years LPN experience. Experience with individuals with intellectual disabilities preferred.  Knowledge of OPWDD rules and regulations. A Valid, non-restricted driver’s license. 
Physical Requirements:
PHYSICAL REQUIREMENTS

When necessary, assist staff with individual physical needs related to medical condition.  The employee must be able to lift and/or move up to 25 pounds.
