The LPN will provide assessment services to children in foster care in the support of pediatric care provided by our pediatricians and will also be responsible for the care coordination services. The individual in this position will report to the Nursing Supervisor (RN) and will receive clinical consultation support from the Chief Physician Duties & Responsibilities: Conduct assessments of children in foster care, including: HIV risk assessments, intake assessments, general first aid and triage activities. Routine screening for: substance abuse, developmental health, mental health and physical body checks for abuse, outline management and training regarding chronic conditions, such as diabetes and asthma.Establish treatment goals for children, utilizing historical and current available information in collaboration with multi-disciplinary team. Education & Experience Associate's Degree Required.  NYS LPN Licensed in NYS is required. One (1) year of experience working with children and adolescents involved in child welfare.  Ability to facilitate training or coaching around medical conditions and treatment. Physical Requirements: While performing the duties for this job, the employee is regularly required to communicate clearly and effectively.  The employee is occasionally required to site and stoop, kneel, crouch or climb stairs.  Must be able to travel unassisted and to use a computer.
