Basic Job Assignment:

*SEEKING TO FILL THREE CASE MANAGER POSITIONS*

Department Program: Healing Oaks Clinic 286 S Euclid Avenue Suite 102 San Diego, CA 92114

POSITION PURPOSE:
The Case Managers help their clients with recovery from substance use and co-occurring disorders through role modeling, and empathic trauma-informed support. They focus on linking to addiction treatment, community resources, transportation and other services the client may need.

 

REPORTING RELATIONSHIP: The SUD Case Managers reports to the SUDLC.

 

PROGRAM SUMMARY FOR HEALING OAKS CLINIC- SUBSTANCE USE DISORDER (SUD) TREATMENT PROGRAM

The Healing Oaks Substance Use Disorder Treatment Program provides outpatient substance use disorder (SUD) treatment, recovery, and ancillary services to adults aged 18 years and above, with substance use issues, including co-occurring mental health disorders. Services includes client and family centered, individualized, clinically effective, outcomes-driven, culturally competent, gender specific, trauma informed, developmentally appropriate, and inclusive of evidence-based treatment practices and recovery service approaches. Co-occurring disorder services includes clinical assessments for substance use and mental health conditions, care coordination and a “warm handoff” to appropriate level of care in the mental health system when need exceeds services available through the SUD program. Evidence based practices is utilized for all clients, including use of specific EPBs that demonstrate success for justice-involved populations as appropriate.

 

 



Employment Requirements:

EDUCATION AND WORK EXPERIENCE: A bachelor’s degree or higher in the field of social work, behavioral sciences, psychology, or a related field and one (1) years of progressively responsible experience in a SUD setting; or an equivalent combination of training and experience.

 

LICENSES, CERTIFICATIONS & SPECIAL REQUIREMENTS: A valid and active Certified SUD Counselor or Licensed Practitioner of the Healing Arts or License Eligible Practitioner of the Healing Hearts. A Valid Driver’s License with current California automobile insurance; A Valid Cardiopulmonary Resuscitation (CPR) Certification; Negative TB test within 60 days prior to hire date or within 7 days after being hired; TB tests are required at the time of hire and annually thereafter; Proof of Completed Coronavirus (COVID-19) Vaccination is required upon hire.

 



Qualifications:

KNOWLEDGE, SKILLS, AND ABILITIES: Knowledge of social, healthcare, supportive and other services/resources available in the San Diego community; Establish and maintain effective working relationships with clients, family members, other professional staff, and the public. Evidence-based practices and issues pertaining to psychosocial treatments for SUD, Motivational Interviewing, and Relapse Prevention. Knowledge of working with adults and families with co-occurring disorder, perinatal services for women and girls, gender responsive treatment, deaf and hard of hearing clients, adolescent services, clients involved with justice system, homeless population, LGBTQ+, veterans, and tribal communities. Experience working with a culturally diverse population and knowledge of cultural issues, values, & beliefs of the target population of adults w/ SMI, homelessness, co-occurring disorders, and criminogenic risks and needs, relevant to providing appropriate SUD program services; Exercise sound, expert independent judgment within general policy guidelines; Communicate clearly and concisely; operate a computer using word processing, spreadsheet, and database software applications; Operate standard office equipment.


Example of Major Functions:

ESSENTIAL RESPONSIBILITIES AND DUTIES: Responsible for case management services and coordination of care; Connect clients with physical health and mental health, transportation, housing, vocational, education, and transition services for reintegration into the community; Services may be in the office or field and require providing transportation to clients; Supervises the day-to-day flow of care coordination activities; Provides trauma informed intensive case management for severely medically frail and/or mentally ill; Provides direct service in areas including but not limited to self-care skills, adaptive coping, education of the process of illness and recovery, social skills enhancement, healthy lifestyle maintenance, vocational rehabilitation as appropriate and advocacy as deemed appropriate; Provides therapeutic, rehabilitative, and recovery services which shall be determined according to the needs of the client as deemed necessary; Develops and implements individual care plans addressing the identified individual biopsychosocial needs and services necessary; Conducts discharge planning to appropriate long-term transitional or permanent housing, including referrals and application assistance; Responds to crisis situations and provides access to emergency care as needed; Establishes and maintains collaborative professional relationships with community providers and other referral sources; Transport clients using company or private vehicle or accompanies clients to appointments in the community as deemed necessary.
