Salary Range:
$17.50-19.00
● Assists with or provides personal care duties as assigned by the nurse or appropriate personnel.● Personal care duties include assistance in: o Bathing in bed, tub, or shower o Care of hair- shampooing o Shaving o Care of the teeth and mouth o Care of nails o Toileting- bed pan, commode, toilet, incontinent care o Dressing o Transferring o Walking o Simple range of motion exercises o Skin Care● Asses and record temperature, pulse, respiration, blood pressure, and weight when assigned.● Perform light housekeeping duties as it relates to the patient. o Making and changing a bed o Tidying bedroom and bathroom● Maintains clinical records as directed.● Reports any abnormal conditions to the professional Nurse or Home Services Assistant Director orDirector● Assists the patient in following medical recommendations under the guidance of the Nurse.● Collaborates to implement the nursing and medical plan as directed: o Assistance with simple, non-sterile dressings such as Tegaderm or Duoderm o Measuring and recording intake and output o Using devices as instructed to aid in normal living activities o Teaching in self-care hygiene

Education and/or Experience● High School diploma or general education degree (GED); successful completion of Basic Nurse Aidetraining program and certificate or written document. Successful work experience in a health care settingdealing with patients for at least one (1) year.
Language Skills● Ability to read and comprehend simple instructions, short correspondence, and memos.● Ability to write simple correspondence.● Ability to effectively present information in one-on-one and small group situations to customers, clients,and other employees of the organization.
Mathematical Skills● Ability to add or subtract two digit numbers and to multiply and divide with 10’s and 100’s.● Ability to perform these operations using units of American money and weight measurement, volume,and distance.
Reasoning Ability● Ability to apply common sense understanding to carry out detailed but uninvolved written or oralinstructions.● Ability to deal with problems involving a few concrete variables in standardized situations.
Certificates, Licenses, Registrations● Successful completion of Basic Nurse Aide training program with certificate/written documents.● Current driver’s license.● Current automobile insurance● Finger print-based background check completed and approved.● Must complete and provide documentation of 12 hours of continuing education annually.● Must be CPR certified and maintain certification.
