Position Summary:

The Cass County Health Department is hiring a full time Registered Nurse (RN) to care for patients in their homes as a part of our Home Health and Hospice programs. The RN will be responsible for developing and maintaining good relationships with clients and their families. The nurse will provide care that is consistent with the patient’s needs and will support their goals. We are looking for an experienced RN who can provide quality and comprehensive nursing care.

Salary Range:
$35-$40 per hour based on experience 

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Effectively uses the nursing processes in assessing, planning, counseling, providing and evaluating home health care services.
  • Provides skilled nursing care to patients within the established plan of treatment.
  • Provides case management services to assigned patients to ensure that all services are provided in accordance with the patient care plan.
  • Develops and maintains a clinical record for each assigned patient which reflects delivery of quality and safe home health and hospice care services.
  • Accurately documents all communications related to patient care in the clinical record in a timely manner per agency policy.
  • Maintains continuity of care by coordinating patient care activities with other disciplines; conducts case conference according to agency policy; and documents case conferences in a timely manner.
  • Conducts supervisory visits of home health aides per agency policy.
  • Proficient with computerized documentation systems.
  • Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
  • Maintains patient confidence and protects department operations by complying with all HIPAA standards and keeping information confidential.
  • Participates in on-call and holiday rotation.
  • Participates in department performance improvement activities as assigned.
  • Maintains professional and technical knowledge by attending educational conferences, in services and meetings and reviewing professional publications.
  • Contributes to team effort by accomplishing set goals.
  • Initiates referrals and coordinates patient care activities with other disciplines.
  • Collaborates with patient/family in planning care and establishing goals.
  • Regularly communicates with physicians.
  • Provides direction, instruction and supervision of the home health aide.
  • Initiates appropriate preventative and rehabilitative nursing procedures.
  • Administers medications and treatments as ordered.
  • Provides education to patient and family.
  • Reports to physician when appropriate and documents in clinical chart; initiates appropriate changes or orders when necessary.
  • Submits accurate weekly time, and monthly time and travel reports.
  • Complies with all CCHD operational policies.
  • Complies with all federal, state and local rules and regulations.
  • Attends staff meetings, CPC, and IDT meetings.

Education and/or Experience

  • An Associate’s or Bachelor’s Degree from an accredited nursing program and/or one to two years related experience and/or training; or equivalent combination of education and experience.

Language Skills

  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence and procedure manuals.
  • Ability to effectively present information and respond to question s from groups of managers, clients, customers, and the general public.

Mathematical Skills

  • Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area circumference, and volume.
  • Ability to apply concepts of basic algebra and geometry.

Reasoning Ability

  • Ability to define problems, collect data, establish facts and draw valid conclusions.
  • Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Computer Skills

  • Proficient in the use of electronic health record software.
  • Proficient in the use of the internet.

Certificates, Licenses, Registrations

  • Current Illinois license as a Registered Nurse.
  • Current driver's license.
  • Current automobile insurance.
  • Maintains current CPR certification.
