Overview:


Florida Assertive Community Treatment (FACT) is a dynamic multidisciplinary treatment team dedicated to delivering intensive community-based support to severely mentally ill adult population. The purpose of FACT Vocational Rehab Professional is to be a part of a multidisciplinary treatment team, function as the primary provider of vocational rehabilitation services and performs modeling skills and consultation as well as providing cross training to other staff on the team to enhance the knowledge and skills of the team to develop supported employment approaches with clients in the team. The Vocational Rehab Professional is responsible for completing thorough assessments that assist in developing a specific treatment plan that addresses the barriers and complex needs of adults with serious mental illness (SMI) with an emphasis in promoting vocational skill development. The Voc. Rehab Specialist services are designed to develop and optimize the skills and abilities of the clients for employment and promoting financial independence through individual and group work.

Shift:  Monday-Friday, 8:00am-5:00pm, with occasional on-call

Rate of Pay:  $24.00-$26.00 per hour

Will cover Wakulla and Gadsden Counties

Education and Experience:


Education/Experience:
• Master’s degree in rehabilitation counseling preferred.
• One year experience providing vocational rehabilitation services.
• One year of professional experience working with persons with severe mental illness (SMI).
• One year of working in outpatient setting experience preferred.
• Assertive community treatment experience working in off-site community settings consistent with the PACT model of service delivery preferred.
• Valid Driver's license with no more than 6 points over 3 years.
