Overview:


Mental Health Targeted Case Managers assist individuals (recipients) in gaining access to needed medical, social, educational, and other services, optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. In addition, services and service frequency should accurately reflect the individual needs, goals, and abilities of each recipient.

Provide case management services, including assessing clients' needs, conducting psychosocial evaluations, developing individual treatment plans, and maintaining daily progress notes.

Provide education and support to clients and family members.

Make appropriate referrals to outside community agencies.

Monitor all aspects of client care and prepare and maintain client records.

Conduct home visits.

Rate of Pay:  
$23.00-$25.00/hr.

Shift: 8:00 am - 5:00 pm



Education and Experience:




Qualifications:

  • Bachelor's degree from accredited university or college with a major in counseling, social work, psychology, or other human services field and one (1) year of full-time experience working with severely and persistently mentally ill children; 
       OR
  • Bachelor's degree from an accredited university or college with other major, and three (3) years of full-time experience working with severely and persistently mentally ill children and their families. 



To be considered for employment, you will be required to undergo a criminal background check, employment references check, and drug screening. In addition you must have a current and valid driver's license with no more than 6 points. A 3 year driving record will also be required upon hire.
