Overview:

Serves as case management supervisor for assigned programs.  Provides direction to and oversight of Targeted Case Management activities and staff. Ensures ongoing compliance with Joint Commission and Medicaid TCM standards. Responsible for completing SPRs for assigned programs.

Rate of Pay:  $26.00/hr. 


Shift:  Monday - Friday, 8:00am-5:00pm

 

 

Education and Experience:

Education/Experience:  

•  Master's degree with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field preferred and three years of full time or equivalent professional experience serving the target population or   
•  Bachelor’s degree and five years of full time or equivalent professional experience serving the target population.
•  Medicaid and TCM experience and knowledge preferred. 

Licensure or Certification: 

• Maintain Targeted Case Manager Supervisor Certification.
• Must possess current, valid driver’s license and maintain ACI designated auto driver status.
