ADC's coordinates/completesclinical and administrative paperwork related to client movement through thesystem, provides case managementservices including coordinating and linking client to needed communityservices, and provides individualcounseling/therapy.

Serves as amember of the treatment team. Coordinates development and documentation ofClient Service Plan (i.e., Treatment Plan) based upon treatment team andattending physician. Updates Client Service Plan as needed.  Attends daily treatment team. Coordinateswith the Physician as needed.


Coordinates and documents dischargeplanning in terms of referral and linking, aftercare and continuing care. Makesappointments for clients who are being discharged, and documents in recordwhere the client is referred to, who the counselor/case manager is and when theappointment is.  Documents that all dischargeplanning/aftercare information has been written and given to client inaccordance with unit procedure (i.e., completes a note in the record uponclient's discharge regarding follow-up placement and appointments, who theclient left with, prescriptions given, final diagnosis, etc.).


Ensures that all patients have beenoffered a BA 40within 72 hours of admission, and informs the psychiatrist ifthe client has not signed BA 40. Notifies psychiatrist if BA coordinatorinforms them that a BA32 has not been receive by final day of client’s 72 hourstay.

Completes and documents ongoingprogress notes regarding implementation of the treatment plan and dischargeplanning. Documents all case management and direct services, and providesindividual supportive counseling to client as needed.


Completes psychosocial assessment forassigned patients in accordance with the Client Record Handbook guidelines.

Education and Experience:
  • Bachelor’s degree from an accredited university or college with a major in the field of counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or a related human services field.
  • Minimum of one (1) year work experience in a mental health-related field preferred.  Inpatient psychiatric experience preferred.

  • Shift: Days vary / 8:00 a.m. - 5:00 pm, preferably for weekend coverage and some weekdays

    Rate of Pay: $11.87 - $12.50hr

    To be considered for employment, you will be required to undergo a criminal background check, employment references check, and drug screening. In addition you must have a current and valid driver's license with no more than 6 points. A 3 year driving record will also be required upon hire.
