Overview:


Assesses plans, develops, coordinates and provides treatment, rehabilitation and support services to persons with severe and persistent mental illness.  Provides crisis intervention.  Serves as client advocate and liaison by providing outreach and supportive services to clients.  Makes appropriate referrals and consultations both within and outside the agency.  Prepares and maintains client records.




Shift:  Hours will vary on a rotation basis which will include some weekends, evenings, and on-call coverage


Rate of Pay: $26.00-$28.00 per hour

Education and Experience:


Qualifications:

  • Minimum of a Masters Degree from an accredited university or college with a major in psychology, social work, counseling or a related human services field and two (2) years of related professional experience.
  • Experience serving persons with severe and persistent mental illness.
  • Experience providing services in community settings preferred. 
  • Must have valid driver's license with no more than 6 points over 3 years.
