Description:

Support Coordinators provide support and resource identification functions to customers recovering frompsychiatric illnesses under clinical supervision and assists with treatment; may provide substance abuse services,education, support and consultation to families; provides crisis intervention as appropriateand serves as integral member of the crisis counseling, stabilization andtherapy team.

Assists clients in gaining access to needed financialand insurance benefits, employment, medical, social, education, and otherservices.  Provides information andreferrals to assist clients’ with needs such as medical, dental, occupational,residential, social and any other identified barriers to stabilization. Workswith the client and the client’s natural support system to meet the needs ofclients.

Completesthe community support assessment as well as other assessment forms such as, butnot limited to the Bio-Psychosocial Preliminary Assessment, MedicalQuestionnaire, and the Medication / Verification form.  Based on the assessment, develops inconjunction with the client / significant other(s), a service plan to addressthe needs of the client.

MeetsService Productivity standard of at least 75% (or 6 out of 8 work hours) forthe provision of Case Management services in accordance with applicableclinical/quality criteria which are supported by appropriate clinical recorddocumentation which meet Client Record Handbook specifications, SAL documentationrequirements, and Agency business/clinical ethical standards.

Providescoordination of care between service providers, Crisis Stabilization Unit(CSU), and other necessary services. Makes appropriate referrals to community resources to assist clients tomet their needs and goals.

Documentsall services provided in the client record in accordance with the Client RecordHandbook Guidelines.  Progress notes mustbe clearly reflected how the support coordinator’s efforts are linked to theservices and the client’s progress or lack of progress on each client’sgoals.  Maintains at least 85% compliancewith progress note requirements on Clinical Reviews conducted during ratingperiod.  Program case files must beconsistent with Center standards, to ensure compliance with Agency policy,F&F rules and JCAHO standards.

Identifyhigh risk clients and complete suicide risk assessments for all clients duringthe intake and/or at anytime during treatment a client are noted to havesuicide ideation, plans, or an attempt. Reviews risk assessments with the Supervisor and /or MD/ARNP uponcompletion.  Includes suicide risk,ideations, plans, or recent attempt to determine appropriate intervention toensure the safety of the clients. Assists the client in completing the Personal Safety Plan.  Maintains at least 85% compliance withassessment requirements on Clinical Reviews conducted during rating period.

Trainedin SSI/SSDI/ Outreach, Access, and Recovery (SOAR) and assist eligible clientsgaining SSI/SSDI.

Attendsand participates in staff/unit meetings as required.


Education and Experience:
  • Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field.
  • Or other Bachelor's degree from an accredited university or college and one (1) year full time experience working with adults experiencing serious mental illness.
  • Ability to manage a large caseload in the provision of focused, short term services.

Shift: Monday - Friday, 8:00 a.m. - 5:00 p.m.

Rate of Pay: $12.50hr

To be considered for employment, you will be required to undergo a criminal background check, employment references check, and drug screening. In addition you must have a current and valid driver's license with no more than 6 points. A 3 year driving record will also be required upon hire.
