Description:

The purpose of CAT case management services is toassist youth and their families in gaining access to needed medical, social, educational,and other services. The primary goal of case management is to optimize thefunctioning of recipients who have complex needs by coordinating the provisionof quality treatment and support services in the most efficient and effectivemanner. Services and service frequency should accurately reflect the individualneeds, goals, and abilities of each recipient.


Assessments: Complete comprehensive assessment of individual youth strengths andneeds. Include review of family factors important to successful youthfunctioning. Contactsothers (family and other providers, with appropriate consent) to provideinformation for the assessment as appropriate. Updates required assessments in accordance with Client Record Handbookguidelines. Finalizes all documents in Avatar and turn in all assessment within48 hours. 

 

Home Visits: Conducts home visits prior to completion of the intake assessmentprocess to evaluate the safety and well being of the client in the setting theclient resides in unless staffed with supervisor and written justification isprovided in the record as to why the home visit could not be made.  Utilizes laptop as appropriate. Meets atleast 85% compliance with home visit requirements on Clinical Reviews conductedduring rating period.

Client Service Plans:  Developsindividualized service plans that include measurable goals and objectives inpartnership with the youth and their families. Complete service plans and reviews that address both youth and familyfactors. Assist with the implementation of the service plan with emphasis onhelping family members achieve autonomy in meeting youth and family needs.  Updates service plans at least every 3 monthsor at any time significant changes occur in the client’s condition, situation,or circumstances (e.g., hospitalizations, achievement of goals, etc). 

Case Management:  Assistfamilies with establishing a natural support system that provides ongoingsupport during and post program participation. Collaborationacross providers and natural support systems, and advocate and assist the youthand his/her family to build meaningful partnership and address their needs, inorder to help them to function better at home, in school and in the community. Assistsclients in gaining access to needed financial, transportation, employment,medical, social, education, and other services. Works with the client and the client’s natural support system to developand implement the client’s service plan. Follows-up on the client’s services to determine the status of theclient’s services and the effectiveness of the service plan towards enhancingthe client’s inclusion in the community. 

ProgressNotes:  Documents all services providedin the client record in accordance with Client Record Handbook Guidelines.  Documentation must be finalized in Avatarwithin 48 hours from the time the service was rendered. Progress notes must bedetailed and clearly reflect how the case manager’s efforts are linked to theservices and goals in the client’s service plan and the client’s progress orlack of progress relative to the service plan.


Education and Experience:
  • Bachelor's degree from accredited university or college with a major in counseling, social work, psychology, or other human services field. and one (1) year of full-time experience working with behaviorally challenged children and young adults; or
  • Bachelor's degree from an accredited university or college with other major, and two (2) years of full-time experience working with behaviorally challenged children and young adults.

  •  Shift: Monday-Friday  8:00 am - 5:00 pm

     Rate of Pay: $14.00hr


    To be considered for employment, you will be required to undergo a criminal background check, employment references check, and drug screening. In addition you must have a current and valid driver's license with no more than 6 points. A 3 year driving record will also be required upon hire.
